Instrument Rental Agreement 2006-2007

If you are interested in renting an instrument from us, after viewing
our rental plans, please fill out the following form and submit it to us.
To authenticate you as a customer, we require your drivers
license number and a valid credit card number.
If you are uncomfortable sending this information over the internet,
you may print this page and fax or mail it to us. We will call you
within a day or two to discuss the details of your rental.

Rental Agreement

Submission of this completed form indicates that I have read and agree with the following terms and conditions.

  1. All instruments are pre-serviced and are in GOOD PLAYING ORDER prior to delivery.
  2. The care of the instrument is the responsibility of the Lessee and all damage other than normal wear is subject to charge.
  3. Rental payments for the School Term are payable in advance for the full term. Ontario rentals are subject to applicable Provincial Sales Tax and Goods and Services Tax.
  4. Stolen instruments must be reported to the police department and evidence of reporting the theft must be delivered to Harknett Music within 5 days. Repairs to any damaged instruments must be completed by our service department unless otherwise authorized in writing.
  5. Insurance is available and covers up to 100% of the replacement value of the instrument.
    ($50.00 deductible)
  6. I acknowledge the above terms, conditions and value and agree to be responsible for the return of the instrument in good working order excluding normal wear and tear on the due date. I will be responsible for any rental charges which occur due to late return of the instrument, and authorize the use of my credit card for payment of these charges.
